You can fill out this form in advance of your visit to Inner Balance Chiropractic, so that we can get you with the Doctor quickly at your scheduled appointment time.

AUTO ACCIDENTS or WORKERS COMP require different forms that are available at our front desk. If you have any questions please talk with our staff about which form to use.
